Bring a large pot of generously salted water to a rolling boil. Add the pasta and cook according to package directions until al dente (approximately 9-11 minutes). Reserve 1 cup of pasta water before draining. Drain the pasta and set aside.

While the pasta cooks, heat 3 tablespoons of olive oil in a large skillet over medium heat. Add the sausage, breaking it up with a spatula, and cook until browned and cooked through, about 7 minutes.

Add the minced garlic to the skillet. If the skillet seems dry, add a drizzle of olive oil. Cook until fragrant, about 1 minute. Pour in the white wine and use a spoon to scrape up any browned bits from the bottom of the pan.

Simmer the wine until it has reduced by about half, approximately 3-5 minutes. Add the reserved pasta water and half of the Parmesan cheese. Stir to combine.

Pour in the heavy cream and stir well. Season with salt and pepper to taste. Let the sauce simmer for 1 minute, allowing it to thicken slightly. Add the drained pasta to the skillet and toss to coat.

Check for seasoning and adjust as needed. Add the remaining Parmesan cheese and stir well to combine. Serve immediately, garnished with fresh parsley and additional Parmesan cheese.

For an extra layer of flavor, consider adding a pinch of red pepper flakes to the sauce.
If the sauce becomes too thick, add a splash of pasta water to loosen it up. Conversely, if the sauce is too thin, simmer it for a few more minutes to reduce it.
Freshly grated Parmesan cheese is highly recommended for the best flavor and texture.
For a vegetarian option, substitute the sausage with mushrooms or other vegetables.
